Medical Records

Upon admission to DIS, every student must present an up-to-date (less than 6 months old) certificate of health. The statement should be completed according to the student Medical Form. This form requires two(2) Laboratory tests (Blood and Urinalysis), a chest X-ray “or” PPD skin test to Tuberculosis, and a General Examination signed by a physician. The student must also be current in the prescribed immunizations and show evidence of having received them. A photocopy of the immunization record should be included and all the health forms should be handed with original copies. A truthful and accurate statement as to medical and emotional/mental health is also required in the Student Medical Form.

Medication

Students shall not keep or take medicine at school without the knowledge of the school nurse or office. If a student must come to school with medicine, then at the start of the school day such medicine shall be placed in the health office.

The medicine with accompanying written statement must be clearly marked with:
  • The student's name/ grade
  • Correct dosage
  • Uses
  • Parent's signature. Medicine will be kept in the health office during school hours.
For safe administration of routine medications brought in from home, the Medication Authorization Form must be filled out by parents and sent along with the medication.

Illnesses

In order to prevent other students from being unnecessarily exposed to illness, parents are asked to keep the student home or pick up their child from school as soon as possible if he or she:
  • Has a fever of 100°F (37.8°C) or higher
  • Has been vomiting
  • Has symptoms that prevent him or her from participating in school, such as excessive tiredness or lack of appetite, productive coughing, headache, body aches, earache, and severe sore throat.
Children should NOT return to school until fever has been gone for 24 hours WITHOUT medication. If your child is ill and unable to attend school, please call the main office or e-mail your child’s teacher and the school nurse on the first day of the illness. It is helpful if the nature of illness is described, so the nurse can watch for general trends.

Emergency Information

It is important that we have up-to-date phone numbers for each parent/guardian and the phone number of two other people who can act for you, as well as the name and phone number of your doctor and dentist. Any changes in the emergency information as listed on your child's enrollment form should be sent to the school office immediately after being changed. 119 will be contacted immediately and an ambulance will transport the student to the hospital. The school will request that students be taken to Fatima Hospital where DIS has contracted a medical agreement with the International Clinic.
